Hi everyone. I have been a member here for a few weeks and thought I should introduce myself.
Rolo is a ~12 year old Corgi/Jack Russel mix, 23.2lbs, diagnosed with diabetes August 24, 2008. Nothing too exciting in her history - she was on Caninsulin for a while, then changed to Humulin N in October. She was hospitalised with mild ketoacidosis in early November (after a drug resistant UTI), but we have been doing relatively well since then.
Our big adventure is getting her insulin sorted out. After many, many curves, food changes, activity changes, and everything else I could think of, we switched from Humulin N to Lantus & NovoRapid. We just started that on Saturday, so things are a little bumpy, but we're sorting it out.
I started her on 2 shots of the Lantus each day, but took a chance on Monday with only 1 shot, and she is definitely getting at least 24hrs duration from it, so that's good news.
She was initially having a drop on the Lantus about 8-10hrs from injection. I though it might be the peak, but it seemed late for that. When I changed her to 1x Lantus daily, she still had the drop 20-22hrs after injection, so so much for a peak. My theory is that her high protein/moderate fat food was keeping her blood sugar elevated for ~8-10hrs, then it was dropping off after that. I have changed her food from Orijen Sr + Merrick canned to Wellness Core Reduced Fat + Merrick canned (her original food) to see if the reduced protein & fat levels her out a bit more.
She was getting a 1/2U bolus of NovoRapid with the Orijen, but I know she will need more on the Wellness (carbs increased from 18.4% 33.4%), so I'm still trying to get the bolus amount sorted out. She started on 4U Lantus 1x daily, but I upped her to 5.5U daily yesterday since her blood sugar was staying in the high 400s. She came down overnight (I assume she got over her insulin resistance from the high BGs), and I reduced her back to 4.5U until she settles in. This morning she was 182 at breakfast, 214 1hr post, 297 5.5hrs post and 290 6hrs post. Seems reasonable for a decreased dose and a too-small bolus! Once I check her tonight at dinner, I will likely increase her bolus as well. As long as she stays in the 200-300s for now, I will leave her on the 4.5U Lantus, then increase in a few days so I don't overwhelm her.
