  • Uncontrolled night time only blood sugar

Oliver, 9yr old blind diabetic (11.5 units Novilin N) in early stages of kidney disease, is finally regulated for now during the day. But ohhh the night time, over night his blood sugar raises up to 500. With history of uncontrolled night time blood sugar and many trips to ER vet over the last two yrs for 600 blood sugar. He is extremely scheduled , nothing has changed. Blood work, ultrasound, thyroid negative , no symptoms of Cushing. His vet is amazing , we are confused by his night time issues. Google has no info. (Except that humans have hormonal changes at night, which is no help.) why night time irregularity? He sleeps all day, I’ve done day curves then night curves. His day curve will be 100s & 200s , night curves will be high 300s & 499! Vet says it’s not Somogyi effect! Does anyone know what happens over night? Schedule daily - 5:30-5:45 am/pm eat(homemade)/ insulin, then 15 min walk,

Hello friends it’s been awhile. I’m so glad this group is here. Oliver is a 9yr old(next month) terrier mix 23.4 lbs. He is an insulin dependent diabetic (7 1/2 units twice a day)on a homemade diet. Yesterday he was diagnosis’s with stage 1 kidney disease. Oliver’s diabetes had been under control (as well as diabetes can be) for the last 4-6 months. I do curve test every 6-8 wks with random test as his behavior changes. Yesterday’s blood work also showed his glucose was 500 in the middle of the day! Wtf! I’m severely neurotic in his care and keep a notebook for any changes. I’m doing a curve today and with his dr will decide the new amount of insulin. His diet twice a day consist of 6oz lean (93/7) ground meat (drained and rinsed) 2oz each fresh green beans fresh carrots whole grain brown rice. 1 teaspoon organic ground flaxseed. 12oz filtered water ( because several months ago he stopped drinking water, after several dehydration packs and test he still refuse to drink! Now I give him sonic ice several times a day .) Oliver is allergic to chicken and dairy. After his diabetes gets stable his dr will start him on kidney meds. She will also want to change his food to low phosphorus kidney friendly food. Oliver has been on homemade food for 2 yrs and has thrived ! The thought of commercial food is troubling. He was going down hill quick before I took him off commercial food. Any general advice or reference material I can read as well as your prior experiences will help! Thank you!

Hi and welcome . Sounds like a challenging time for you 2 . My Jesse lived 11 years being diabetic on a homemade diet . It's not a common place . She passed away at almost 17 and did not pass from diabetes .

It's hard to know how the kidneys are going to affect blood sugar and that's what you maybe seeing now . A curve is important to see what's really occuring with blood sugar throughout the day . I place little on one number unless it's low ..

You can post the curve to see what your up against
Jesse-26 lbs - 16.5 years old ,11 years diabetic, one meal a day homemade and a vitabone snack . 3 shots of Novolin( under the Relion name ) a day . Total insulin for a 24 hour period is 6.5 units of NPH insulin .

actually i did feed Riliey the same as you feed Oliver
but later found a good wet dog food its not troubling at all. he decided he needed a change
we started over matching insulin to the food doing regular curves.
5 1/2 units and 1/2 cup food twice a day
        easy peezy

Hi and welcome . You could try an experiment and raise the daytime numbers up a bit and see if that lowers the nighttime numbers . You would probably have to lower the daytime dose to do that . Your looking for a bit of balance

It's difficult determine somogy and using text book reasons . You are seeing a big swing from day and night which is rebound in nature .

My Jesse could not accept a full dose of insulin that I had to split at mealtime to have her body be ok with .

Another option is to give a lower dose or higher dose at night . I usually like starting with lower . Sometimes it doesn't take much .

For me with such high swings in blood sugar I lean to the body responding to injected insulin and blood sugar . The problem is we can't test for where sugar comes from

You are half way their having good sugar half the day .

I had to get very creative with my jesses routine . Way outside thinking out of the box .

the 100 to 200 is awesome so increasing his morning dose will further bring down am numbers .
with pm 300 to 500 an increase of .5 at night should bring down nitetime numbers
it's possible a 30 minute walk at nite will decrease

There is a huge difference just in metabolism and activity at night as well so it kind of makes sense that those two 12-hour periods don't look all that much alike. Have you worked with his evening dosing and/or meal time to see if you can shift those night time highs? Or are they sporadic?
