I can't help with what the rounded liver means. Hopefully it isn't anything serious and I will be keeping Baxter in my prayers. (((hugs)))

There is actual a term in human diabetes known as a diabetic liver. The liver plays a large role in glucose management and is affected by diabetes. Often it looks like a "fatty liver." How have Baxter's liver enzymes been on labwork?

The fact that his numbers are fluctuating more can impact the liver's role in management. I wouldn't get overly concerned without more information yet.

As you say, it is possible without his usual walk and/or stress from the clinic his numbers trended higher. If 10u was previously too much and 9u not enough, I'd be inclined to start with 9 1/2u, wait a week then increase again if it's still needed.

            • liver woes need your opinions

Hi vet talked to me again and she mentioned x rays revealed slightly enlarged liver and a nodule with roundness on back of his liver (vet trying to identify why humulin n not as effective as it once was.)they want us to do $500 ultrasound..cost prohibitive right now After broken water heater and $400 xrays. Anyone have experience with liver issues in diabetic dog.we may be able to do ultrasound in a week from now. His insulin upped to 10 twice dailjanuary bloodwork only revealed elevated cholesterol n triglycerides liver values normal. Please help so confused. Thx Jen

Have you done a reading on high tryglycerides? I know those can cause a real problem with regulation. Unfortunately that is all I know about it though. You could google high tryglycerides and diabetes and see what comes up. I believe a special diet can help.

I'm terrified insulin not effective anymore he usually comes down after 2 hours at vet cuve.but that during day.. Vet even increased dose plus added worry of liver enlargement so scared. He did not have any elevatd liver enzymes on bloodwork in feb. This perfectly regulated dog now all over place coukd hi choles n tryglycrides delay humuln n working?? Ultrasound liver friday. I'm just a little confused by your first sentence. Did you inject, then 3 hours later at 7:45 feed and inject another 8 units (two injections within about 3 hours)?

Some pups rise for several hours after a meal and then slowly come down. Has Baxter been active this evening, or a little "lazy"? The reason I'm asking is that my Annie becomes a couch potato after her dinner and her blood glucose might go a little higher because of the inactivity of being on the sofa!

Here is a little blurb about high tryglycerides and the link from the k9diabetes home page for it:

                          http://www.veterinarypartner.com/Con...3055&SourceID=

                          Excess Circulating Lipids

High levels of circulating triglycerides (fats) interfere with binding insulin to the cells it needs to act upon. While most dogs with excess circulating lipids are not diabetic, when a diabetic patient is insulin resistant it may be helpful to attempt to reduce the circulating fats. Excess lipids in the blood usually occur because of some other hormone imbalance (Cushing’s disease, hypothyroidism in dogs etc.) but they can simply happen spontaneously in the miniature schnauzer.

Circulating fats are reduced by treating the hormone imbalance that caused them if there is one. If there is no underlying disease, circulating lipids can be reduced with diet and omega 3 fatty acid supplementation.

I know you are very scared and I hope I don't add to your worry with the above mention of Cushing's disease. Many dogs can be suspected of having Cushing's and when the diabetes is better controlled the Cushing's like symptoms which are very like diabetes symptoms go away.

There are also dogs I know of that have had high tryglycerides and have not had Cushing's. One is a Min Schauzer, but the other was Eileen's beloved Mildred, who I don't believe had Cushing's.

I don't believe you initially tested at home, but also am not up to date on your thread. I am sceptical when folks who don't have the advantage of daily at home testing say the dog was well regulated. This isn't a distrust of you, but more a distrust of the disease as I know how ten us regulation can be in some dogs and I believe your pup hasn't been diabetic for that long.

Is it possible that he was at the vet for some other reason and they happened to discover that things had gone haywire?

I know it is hard to worry, but diabetics can go wildly off track for all sorts of reasons. I hear the panic in your post and that may cause to alter things giving him less consistency. I only recognize this because I have been there.

If you have a chance and could post all the numbers at vets and at home with times of feeding, tests and shots we might be able to shine some light.

The single 198 is great, but I'm afraid you will need to do more testing. This could be the start of a very nice "curve", testing every 2 hours to see how Baxter does throughout the day. Don't take any corrective action unless he drops to 100. If that should happen, then give some sugary item along with a bit of food to stop the drop (the tricky part I had was that I'd give too much and Annie would go from 100 to 300 in an hour). For the sugary item I'm using honey, but have used corn syrup. When I intervene, I smear a small amount of honey on a treat, but every pup is different.

                            HAVE FUN!

Hi, so we are taking baby steps testing...this weekend we will do 3-4 readings today and tomorrow and report to the vet. So, Craig, heres what I need your help with..
                              Today, Baxter starts with
                              fasting bg 8:07 am: 198
                              8:35am ate all meal
                              8:53am 10 units humulin n given
10:00am 1/2 alpo snap after going outside (do you have any info how much [B]treats like this affect bg?)
At 12;30pm today his reading was 350 ???? Please respond to questions below..thx.
                              1. Could this reading still be food processing and the small bit of treat? What is usual onset time for humulin N?
                              2. Puzzled, because he used to get drops after 1 hour when he did earlier curves in like november/december/january..wondering why he processing insulin differently?
                              3. If we're getting a good start number in mornings, it seems overnight all is working as it should, but why hasn't he dropped yet today? We will test again later,
                              4. Have you heard of different day and night processing of insulin???

I'm at Starbucks using my iPhone, so a little hard to type!

So you are up about 150 points from this morning, about 3.5 hours after the injection. Annie use to do this. She would have her highest reading around 4-5 hours after the injection, and then come back down having her lowest reading around the 10th or 11th hour. Will be interesting to see the rest of the day.

I think most of us test, feed and inject right together. I "start" Annie at 7:30 and am done by 7:45ish. You didn't inject until nearly an hour after the BG reading, so he could have been in the mid 200s by that time?

I'm not familiar with the treat you gave, but some snacks can raise the BG. I use 1-2 pieces of frozen green bean as treats. Annie lives to play ball, so often instead of a food treat I'll throw her ball a few times.

Night & day. Yes, it seems the lower activity (couch potato, sleeping) at night causes a slightly different insulin response in some pups. I frequently give 1/4 unit more with the evening meal. Every dog / situation is different.

Onset of Novolin-N seems to vary with the pup. As I mentioned above, Annie seems to take maybe 4 hours, but others report a faster onset. Think there are many factors: amount of insulin, injection site, food, activity and probably much more.

This has taken about 30 minutes for me type by one finger on the iPhone. Lets see if anyone else answered.

                                Craig
